Abstract Objective: to evaluate how different educational strategies contribute to knowledge gains perceived by caregivers of people using Enteral Nutritional Therapy. Method: a quasi-experimental study conducted in two stages: the first one included an interactive lecture class (LC) and the second was carried out in two groups: in-situ simulated skills training (ST) and reading of an educational booklet (EB). The caregivers answered a self-administered questionnaire to assess knowledge before and after the interventions; for the analysis, a generalized linear model with Poisson distribution was proposed and the comparisons were carried out using orthogonal contrasts. Results: the participants were 30 caregivers; evidence of a difference in knowledge between the t1and t0 moments is evidenced. The analysis of the final comparison about the knowledge gain between the EB and ST groups, according to Student's t, evidenced an estimated difference of -1,33, with 95% CI (-4.98; 2.31) and p-value=0.46. Conclusion: knowledge was further increased between the t1 and t0 moments, when compared to the t2 and t1 moments in both groups. When compared, we cannot conclude that one of the groups changed more than the other in relation to moment t0 and t2; thus, the study evidenced the knowledge gain after all the educational strategies in both groups.


Resumo Objetivo: avaliar como diferentes estratégias educativas contribuem para ganhos de conhecimento percebidos por cuidadores de pessoas em uso da Terapia Nutricional Enteral. Método: estudo quase-experimental realizado em duas etapas; a primeira contemplou uma aula expositiva dialogada (AE) e a segunda aconteceu em dois grupos: treino de habilidades (TH) simulado in situ e leitura da cartilha educativa (CE). Os cuidadores responderam um questionário autoaplicável para avaliação de conhecimentos em pré e pós-intervenções; para a análise foi proposto um modelo linear generalizado com distribuição Poisson e as comparações foram realizadas por contrastes ortogonais. Resultados: participaram 30 cuidadores, observou-se evidência de diferença de conhecimento entre os tempos t1 e t0. A análise da comparação final sobre o aumento do conhecimento entre os grupos CE e TH, por teste t-Student, evidenciou uma diferença estimada de -1,33, com IC 95% (-4,98; 2,31) e valor de p de 0,46. Conclusão: ocorreu uma maior elevação de conhecimento entre os tempos t1 e t0, quando comparada os tempos t2 e t1 em ambos os grupos. Quando comparados, não podemos concluir que um dos grupos mudou mais que o outro em relação aos tempos t0 e t2; assim, o estudo evidenciou o ganho de conhecimento após todas as estratégias educativas nos dois grupos.


Resumen Objetivo: evaluar cómo las diferentes estrategias educativas contribuyen a la adquisición de conocimiento percibida por los cuidadores de personas que utilizan Terapia Nutricional Enteral. Método: estudio cuasiexperimental realizado en dos etapas; la primera incluyó una clase expositiva dialogada (CE) y la segunda se desarrolló en dos grupos: entrenamiento de habilidades (EH) simuladas in situ y lectura del folleto educativo (FE). Los cuidadores respondieron un cuestionario autoadministrado para evaluar el conocimiento pre-posintervenciones; para el análisis se propuso un modelo lineal generalizado con distribución de Poisson y las comparaciones se realizaron mediante contrastes ortogonales. Resultados: participaron 30 cuidadores, había evidencias de la diferencia de conocimiento entre los tiempos t1 y t0. El análisis de la comparación final sobre el aumento de conocimientos entre los grupos FE y EH, mediante la prueba t de Student, mostró una diferencia estimada de -1,33, con un IC del 95% (-4,98; 2,31) y un valor de p de 0,46. Conclusión: hubo un mayor aumento del conocimiento entre los tiempos t1 y t0, que entre los tiempos t2 y t1 en ambos grupos. Al compararlos, no podemos concluir que uno de los grupos cambió más que el otro entre t0 y t2; por lo tanto, el estudio demostró que hubo adquisición de conocimiento después de todas las estrategias educativas en ambos grupos.

OBJECTIVE: To analyze the spatial behavior of hepatitis A, measles, mumps, and rubella (MMR), and varicella vaccination coverage in children and its relationship with socioeconomic determinants in the state of Minas Gerais. METHODS: This ecological study investigated records of doses administered to children, extracted from the Immunization Information System of 853 municipalities in Minas Gerais, in 2020. We analyzed the vaccination coverage and socioeconomic factors. Spatial scan statistics were used to identify spatial clusters and measure the relative risk based on the vaccination coverage indicator and the Bivariate Moran Index, and thus detect socioeconomic factors correlated with the spatial distribution of vaccination. We used the cartographic base of the state and its municipalities and the ArcGIS and SPSS software programs. RESULTS: Hepatitis A (89.0%), MMR (75.7%), and varicella (89.0%) showed low vaccination coverage. All vaccines analyzed had significant clusters. The clusters most likely to vaccinate their population were mainly located in the Central, Midwest, South Central, and Northwest regions, while the least likely were in the North, Northeast, and Triângulo do Sul regions. The municipal human development index, urbanization rate, and gross domestic product were spatially dependent on vaccination coverage. CONCLUSIONS: The spatial behavior of hepatitis A, MMR, and varicella vaccination coverage is heterogeneous and associated with socioeconomic factors. We emphasize that vaccination records require attention and should be continuously monitored to improve the quality of information used in services and research.

RESUMO Objetivo: Analisar o comportamento espacial da cobertura vacinal da hepatite A, tríplice viral e varicela em crianças e a relação com determinantes socioeconômicos, no estado de Minas Gerais. Métodos: Estudo ecológico que considerou os registros de doses aplicadas em crianças extraídos do Sistema de Informação de Imunização de 853 municípios mineiros, em 2020. Analisaram-se as coberturas vacinais e os fatores socioeconômicos. A estatística scan espacial foi utilizada para identificar agrupamentos espaciais e medir o risco relativo pelo indicador de cobertura vacinal e o Índice de Moran Bivariado para identificar os fatores socioeconômicos correlacionados com a distribuição espacial da vacinação. Utilizou-se a base cartográfica do estado e de seus municípios e os softwares ArcGIS e SPSS. Resultados: Observaram-se baixas coberturas para as vacinas de hepatite A (89,0%), tríplice viral (75,7%) e varicela (89,0%). Foram identificados aglomerados significativos em todas as vacinas analisadas. Os aglomerados de maior chance de a população ser vacinada se alocaram majoritariamente nas regiões Centro, Centro-Oeste, Centro-Sul, Noroeste, e as de menor propensão para a vacinação encontram-se nas regiões Norte, Nordeste e Triângulo do Sul. O índice de desenvolvimento humano municipal, a taxa de urbanização e o produto interno bruto tiveram dependência espacial com a cobertura vacinal. Conclusões: Há heterogeneidades no comportamento espacial das coberturas para as vacinas hepatite A, tríplice viral e varicela, e essa condição se encontra associada a fatores socioeconômicos. Destaca-se que os registros de vacinação requerem atenção e devem ser continuamente acompanhados para aprimoramento da qualidade das informações utilizadas nos serviços e nas pesquisas.

OBJECTIVE: to identify spatial clusters corresponding to abandonment of routine vaccines in children. METHOD: an ecological study, according to data from the 853 municipalities of a Brazilian state. The records analyzed were those of the multidose pentavalent, pneumococcal 10-valent, inactivated poliomyelitis and oral human rotavirus vaccines of 781,489 children aged less than one year old. The spatial scan statistics was used to identify spatial clusters and assess the relative risk based on the vaccination abandonment indicator. RESULTS: the spatial scan statistics detected the presence of statistically significant clusters for abandonment regarding the four vaccines in all the years analyzed. However, the highest number of clusters with high relative risk estimates was identified in 2020. The Vale do Aço and West, North and West, and Southwest regions stand out for the pentavalent, poliomyelitis and rotavirus vaccines, respectively. CONCLUSION: in an attempt to mitigate the devastating impact of the COVID-19 pandemic, the immunization program experienced setbacks. The presence of clusters points to the need to implement integrated strategies that may involve different sectors for an active search for children and prevent outbreaks of vaccine-preventable diseases in the near future.

OBJECTIVES: To identify risk-prone areas for the spread of tuberculosis, analyze spatial variation and temporal trends of the disease in these areas and identify their determinants in a high burden city. METHODS: An ecological study was carried out in Ribeirão Preto, São Paulo, Brazil. The population was composed of pulmonary tuberculosis cases reported in the Tuberculosis Patient Control System between 2006 and 2017. Seasonal Trend Decomposition using the Loess decomposition method was used. Spatial and spatiotemporal scanning statistics were applied to identify risk areas. Spatial Variation in Temporal Trends (SVTT) was used to detect risk-prone territories with changes in the temporal trend. Finally, Pearson's Chi-square test was performed to identify factors associated with the epidemiological situation in the municipality. RESULTS: Between 2006 and 2017, 1760 cases of pulmonary tuberculosis were reported in the municipality. With spatial scanning, four groups of clusters were identified with relative risks (RR) from 0.19 to 0.52, 1.73, 2.07, and 2.68 to 2.72. With the space-time scan, four clusters were also identified with RR of 0.13 (2008-2013), 1.94 (2010-2015), 2.34 (2006 to 2011), and 2.84 (2014-2017). With the SVTT, a cluster was identified with RR 0.11, an internal time trend of growth (+ 0.09%/year), and an external time trend of decrease (- 0.06%/year). Finally, three risk factors and three protective factors that are associated with the epidemiological situation in the municipality were identified, being: race/brown color (OR: 1.26), without education (OR: 1.71), retired (OR: 1.35), 15 years or more of study (OR: 0.73), not having HIV (OR: 0.55) and not having diabetes (OR: 0.35). CONCLUSION: The importance of using spatial analysis tools in identifying areas that should be prioritized for TB control is highlighted, and greater attention is necessary for individuals who fit the profile indicated as "at risk" for the disease.

Objective: To identify factors correlated with the incidence and mortality from COVID-19 and investigate syndemic situations at the global level. Method: An ecologic study of confirmed COVID-19 cases and deaths was performed using information collected from the European Center for Disease Prevention and Control in 2019 and 2020. World Bank indicators and information obtained from Worldometer Coronavirus were used to characterize the countries. Descriptive analyses and correlations between independent variables were performed, followed by multiple linear regression analysis to identify factors correlated with COVID-19 incidence and mortality. Results: Data were obtained for 185 countries. Mean case incidence was 16 482/1,000 population, whereas mean COVID-19 mortality was 291/1,000 population, with the highest and lowest rates recorded in North America and East Asia and Pacific respectively. A positive correlation was identified between incidence rate and percent population aged 15 to 64 years, urban population, inequality measured by the Gini coefficient, and six out of the seven regions analyzed (except East Asia and Pacific). Mortality rate was negatively correlated with population aged 0 to 14 years and positively correlated with urban population, inequality measured by the Gini coefficient, and all regions analyzed except East Asia and Pacific. Conclusions: COVID-19 morbidity and mortality were correlated with the burden of chronic diseases, aging population, and low capacity of healthcare services for testing and providing hospital beds, a scenario complicated by social inequality in countries and regions, indicating a syndemic effect.

Métodos: Se realizó un estudio ecológico de casos de COVID-19 y de defunciones confirmadas por esa causa a partir de la información obtenida del Centro Europeo para la Prevención y el Control de las Enfermedades en el 2019 y el 2020. Para caracterizar a los países, se utilizaron indicadores del Banco Mundial y del sitio web de referencia Worldometer Coronavirus. Se hicieron análisis descriptivos y de correlación entre las variables independientes para crear posteriormente un modelo de regresión lineal múltiple con el fin de identificar los factores correlacionados con la incidencia de COVID-19 y la mortalidad por esa causa. Resultados: Se obtuvieron datos de 185 países. La tasa media de incidencia de casos de COVID-19 fue de 16 482 por mil habitantes y la tasa media de mortalidad por esa causa fue de 291 por mil habitantes. Las regiones de América del Norte y de Asia oriental y el Pacífico presentaron los mayores y menores índices, respectivamente. Se observó una correlación positiva de la tasa de incidencia con la proporción del grupo de 15 a 64 años de edad, la población urbana, la desigualdad medida por el coeficiente de Gini y seis de las siete regiones analizadas (excepto Asia oriental y el Pacífico). La tasa de mortalidad presentó una correlación negativa con el grupo de 0 a 14 años de edad y positiva con la población urbana, la desigualdad medida por el coeficiente de Gini y todas las regiones analizadas, excepto Asia oriental y el Pacífico. Conclusiones: La morbimortalidad por COVID-19 guardó una correlación con la carga de problemas crónicos de salud, el envejecimiento de la población y la poca capacidad de realizar pruebas en los servicios de salud y de ofrecer camas de hospital, cuadro agravado en los países o regiones con una elevada tasa de desigualdad social y característico de una situación de sindemia.

Resumo Introdução A mortalidade por tuberculose é um indicador sensível às desigualdades em saúde, considerando-se que o risco para sua ocorrência está relacionado à vulnerabilidade dos territórios. Objetivo Investigar se as variações espaciais dos determinantes sociais afetam as mortes por tuberculose em um município da região amazônica. Método Estudo ecológico com medidas múltiplas de análise, composto por óbitos por tuberculose como causa básica em Manaus, Amazonas, Brasil, notificados no Sistema de Informação sobre Mortalidade (2006-2015). Nas análises, foram calculadas as taxas bruta e padronizada, e usado o Índice de Moran para verificar a autocorrelação espacial da mortalidade e, posteriormente, foi aplicada a regressão geograficamente ponderada para aferir a relação da desigualdade com as mortes por tuberculose. Resultados Foram identificados 731 óbitos pela doença. A distribuição da mortalidade ocorreu de forma heterogênea, sendo que as altas taxas de óbito por tuberculose estavam em áreas mais socialmente vulneráveis. Foram confirmadas a autocorrelação e a dependência espacial. O modelo final apresentou as variáveis indicadoras de iniquidades (baixa renda, pobreza e escolaridade), mostrando relação destas com a mortalidade. Conclusão Ficou evidente que os determinantes sociais no espaço urbano influenciam a mortalidade por tuberculose na região. Portanto, avançar em políticas públicas para corrigir as iniquidades em saúde pode influenciar positivamente esse cenário.


Abstract Background Mortality from tuberculosis is a sensitive indicator of health inequalities, given that the risk for its occurrence is related to the vulnerability of the territories. Objective To investigate whether the spatial variations of social determinants affect deaths from tuberculosis in a municipality in the Amazon region. Method Ecological study with multiple measures of analysis. The study was composed of deaths from tuberculosis as a basic cause in Manaus, Amazonas, Brazil, reported in the Brazilian Mortality Information System (2006-2015). In the analyzes, the crude and standardized rate was calculated, Moran Index was used to verify the spatial autocorrelation of mortality, and subsequently, the geographically weighted regression was applied to assess the inequality relationship of deaths from tuberculosis. Results 731 deaths from the disease were identified. The distribution of mortality was heterogeneous, with high rates of death from tuberculosis in more socially vulnerable areas. The autocorrelation and spatial dependence was confirmed. The final model presented variables that indicate inequities: low income, poverty and education, showing their relationship with mortality. Conclusion It was evident that social determinants in the urban space influence mortality from tuberculosis in the region. Therefore, advancing public policies to correct health inequities can positively impact this scenario.

[RESUMO]. Objetivo. Identificar os fatores correlacionados à incidência e mortalidade por COVID-19 e verificar situações de sindemia em escala global. Métodos. Realizou-se um estudo ecológico de casos e óbitos confirmados de COVID-19 a partir de informações coletadas do European Center for Disease Prevention and Control em 2019 e 2020. Para a caracterização dos países, utilizaram-se indicadores do Banco Mundial e Worldometer Coronavirus. Foram realizadas análises descritivas e de correlação entre as variáveis independentes para posteriormente realizar o modelo de regressão linear múltipla, com o objetivo de identificar os fatores correlacionados à incidência e mortalidade por COVID-19. Resultados. Obtiveram-se dados de 185 países. A média da incidência dos casos foi de 16 482/mil habitantes, enquanto a média para mortalidade por COVID-19 foi de 291/mil habitantes, sendo América do Norte e Leste Asiático e Pacífico as regiões que apresentaram maiores e menores índices, respectivamente. Identificouse correlação positiva da taxa de incidência com proporção da população com idade de 15 a 64 anos, população urbana, desigualdade conforme Índice de Gini e com seis das sete regiões analisadas (exceto Leste Asiático e Pacífico). A taxa de mortalidade apresentou correlação negativa com a população de 0 a 14 anos e positiva com população urbana, desigualdade conforme índice de Gini e todas as regiões analisadas, exceto Leste Asiático e Pacífico. Conclusões. A morbimortalidade da COVID-19 esteve correlacionada à carga de condições crônicas, ao envelhecimento da população e à baixa capacidade dos serviços de saúde para testagem e oferta de leitos hospitalares, quadro agravado em países ou regiões com elevada desigualdade social, caracterizando uma situação de sindemia.

Se observó una correlación positiva de la tasa de incidencia con la proporción del grupo de 15 a 64 años de edad, la población urbana, la desigualdad medida por el coeficiente de Gini y seis de las siete regiones analizadas (excepto Asia oriental y el Pacífico). La tasa de mortalidad presentó una correlación negativa con el grupo de 0 a 14 años de edad y positiva con la población urbana, la desigualdad medida por el coeficiente de Gini y todas las regiones analizadas, excepto Asia oriental y el Pacífico. Conclusiones. La morbimortalidad por COVID-19 guardó una correlación con la carga de problemas crónicos de salud, el envejecimiento de la población y la poca capacidad de realizar pruebas en los servicios de salud y de ofrecer camas de hospital, cuadro agravado en los países o regiones con una elevada tasa de desigualdad social y característico de una situación de sindemia.

Abstract Objective: to identify spatial clusters corresponding to abandonment of routine vaccines in children. Method: an ecological study, according to data from the 853 municipalities of a Brazilian state. The records analyzed were those of the multidose pentavalent, pneumococcal 10-valent, inactivated poliomyelitis and oral human rotavirus vaccines of 781,489 children aged less than one year old. The spatial scan statistics was used to identify spatial clusters and assess the relative risk based on the vaccination abandonment indicator. Results: the spatial scan statistics detected the presence of statistically significant clusters for abandonment regarding the four vaccines in all the years analyzed. However, the highest number of clusters with high relative risk estimates was identified in 2020. The Vale do Aço and West, North and West, and Southwest regions stand out for the pentavalent, poliomyelitis and rotavirus vaccines, respectively. Conclusion: in an attempt to mitigate the devastating impact of the COVID-19 pandemic, the immunization program experienced setbacks. The presence of clusters points to the need to implement integrated strategies that may involve different sectors for an active search for children and prevent outbreaks of vaccine-preventable diseases in the near future.


Resumo Objetivo: identificar aglomerados espaciais de abandono de vacinas de rotina em crianças. Método: estudo ecológico, segundo dados dos 853 municípios de um Estado brasileiro. Foram analisados registros das vacinas multidoses pentavalente, pneumocócica 10-valente, vacina inativada contra a poliomielite e vacina oral de rotavírus humano de 781.489 crianças menores de um ano de idade. A estatística scan espacial foi utilizada para identificar agrupamentos espaciais e medir o risco relativo a partir do indicador de abandono de vacinas. Resultados: a estatística scan espacial detectou a presença de aglomerados estatisticamente significativos para o abandono das quatro vacinas em todos os anos analisados. No entanto, o maior número de aglomerados com elevadas estimativas dos riscos relativos foi identificado no ano de 2020. Destaca-se as macrorregiões do Vale do Aço e Oeste; Norte e Oeste; e Sudeste para as vacinas pentavalente, poliomielite e rotavírus, respectivamente. Conclusão: na tentativa de mitigar o impacto devastador da pandemia de COVID-19, o programa de imunização retrocedeu. A presença de aglomerados aponta a necessidade de implementar estratégias integradas que possam envolver diferentes setores para a busca ativa de crianças e evitar surtos de doenças imunopreveníveis no futuro próximo.


Resumen Objetivo: identificar grupos espaciales que abandonaron la vacunación de rutina de los niños. Método: estudio ecológico, basado en los datos de 853 municipios de un Estado brasileño. Se analizaron los registros de vacunas multidosis pentavalente, antineumocócica 10-valente y antipoliomielítica inactivada y vacuna oral contra el rotavirus humano de 781.489 niños menores de un año de edad. Se utilizó la estadística scan espacial para identificar agrupaciones espaciales y medir el riesgo relativo del indicador abandono de la vacunación. Resultados: la estadística scan espacial detectó la presencia de grupos estadísticamente significativos para el abandono de las cuatro vacunas en todos los años analizados. Sin embargo, el mayor número de grupos con estimaciones altas de riesgos relativos se identificó en 2020. Se destacan las macrorregiones del Vale do Aço y Oeste; Norte y Oeste; y Sudeste para las vacunas pentavalente, antipoliomielítica y contra el rotavirus, respectivamente. Conclusión: mientras se intentaba disminuir el impacto devastador de la pandemia de COVID-19, retrocedió el programa de inmunización. La presencia de grupos indica que es necesario implementar estrategias integradas que puedan involucrar a diferentes sectores para la búsqueda activa de niños y evitar brotes de enfermedades inmunoprevenibles en el futuro próximo.

RESUMO Objetivo. Identificar os fatores correlacionados à incidência e mortalidade por COVID-19 e verificar situações de sindemia em escala global. Métodos. Realizou-se um estudo ecológico de casos e óbitos confirmados de COVID-19 a partir de informações coletadas do European Center for Disease Prevention and Control em 2019 e 2020. Para a caracterização dos países, utilizaram-se indicadores do Banco Mundial e Worldometer Coronavirus. Foram realizadas análises descritivas e de correlação entre as variáveis independentes para posteriormente realizar o modelo de regressão linear múltipla, com o objetivo de identificar os fatores correlacionados à incidência e mortalidade por COVID-19. Resultados. Obtiveram-se dados de 185 países. A média da incidência dos casos foi de 16 482/mil habitantes, enquanto a média para mortalidade por COVID-19 foi de 291/mil habitantes, sendo América do Norte e Leste Asiático e Pacífico as regiões que apresentaram maiores e menores índices, respectivamente. Identificouse correlação positiva da taxa de incidência com proporção da população com idade de 15 a 64 anos, população urbana, desigualdade conforme Índice de Gini e com seis das sete regiões analisadas (exceto Leste Asiático e Pacífico). A taxa de mortalidade apresentou correlação negativa com a população de 0 a 14 anos e positiva com população urbana, desigualdade conforme índice de Gini e todas as regiões analisadas, exceto Leste Asiático e Pacífico. Conclusões. A morbimortalidade da COVID-19 esteve correlacionada à carga de condições crônicas, ao envelhecimento da população e à baixa capacidade dos serviços de saúde para testagem e oferta de leitos hospitalares, quadro agravado em países ou regiões com elevada desigualdade social, caracterizando uma situação de sindemia.


ABSTRACT Objective. To identify factors correlated with the incidence and mortality from COVID-19 and investigate syndemic situations at the global level. Method. An ecologic study of confirmed COVID-19 cases and deaths was performed using information collected from the European Center for Disease Prevention and Control in 2019 and 2020. World Bank indicators and information obtained from Worldometer Coronavirus were used to characterize the countries. Descriptive analyses and correlations between independent variables were performed, followed by multiple linear regression analysis to identify factors correlated with COVID-19 incidence and mortality. Results. Data were obtained for 185 countries. Mean case incidence was 16 482/1,000 population, whereas mean COVID-19 mortality was 291/1,000 population, with the highest and lowest rates recorded in North America and East Asia and Pacific respectively. A positive correlation was identified between incidence rate and percent population aged 15 to 64 years, urban population, inequality measured by the Gini coefficient, and six out of the seven regions analyzed (except East Asia and Pacific). Mortality rate was negatively correlated with population aged 0 to 14 years and positively correlated with urban population, inequality measured by the Gini coefficient, and all regions analyzed except East Asia and Pacific. Conclusions. COVID-19 morbidity and mortality were correlated with the burden of chronic diseases, aging population, and low capacity of healthcare services for testing and providing hospital beds, a scenario complicated by social inequality in countries and regions, indicating a syndemic effect.

Se realizó un estudio ecológico de casos de COVID-19 y de defunciones confirmadas por esa causa a partir de la información obtenida del Centro Europeo para la Prevención y el Control de las Enfermedades en el 2019 y el 2020. Para caracterizar a los países, se utilizaron indicadores del Banco Mundial y del sitio web de referencia Worldometer Coronavirus. Se hicieron análisis descriptivos y de correlación entre las variables independientes para crear posteriormente un modelo de regresión lineal múltiple con el fin de identificar los factores correlacionados con la incidencia de COVID-19 y la mortalidad por esa causa. Resultados. Se obtuvieron datos de 185 países. INTRODUCTION: Prisons are high-risk settings for drug-resistant tuberculosis because the prevalence of the tuberculosis (TB) is much higher than in the general population. This study to investigated the factors associated with drug-resistant tuberculosis in prisons in the state of São Paulo, Brazil. METHODOLOGY: Retrospective cohort of drug-resistant TB cases for incarcerated people in São Paulo state, reported in the Tuberculosis Patient Control System between 2006 and 2016. To analyze the factors associated with drug-resistant TB, the backward method (likelihood ratio) was used, determining the adjusted odds ratio and respective 95%CI coefficients. Multiple models were proposed to adjust for potential confusion and interaction. The best fit model was selected based on the lowest Akaike information criterion coefficient. RESULTS: In total, 473 drug-resistant tuberculosis cases were reported in the prison population of Sao Paulo state, the majority were male. The cases that presented negative results for sputum smear and sputum culture had, respectively, an aOR=0.6 and aOR=0.16 for drug-resistant tuberculosis in relation to the cases with positive results. The cases where the patient had AIDS and reported alcoholism, respectively, an aOR=1.47 and aOR=1.60 for drug-resistant TB. Individuals with a background treatment history for TB presented a stronger association with drug-resistant tuberculosis, aOR=35.08. CONCLUSIONS: Sputum spear, sputum culture, chest X-ray, AIDS, alcoholism and background treatment history for TB were factors associated with resistance to antituberculosis drugs among prisoners. This is useful for the implementation of disease control measures related to the detection and monitoring of cases in the prison system.

BACKGROUND: The aim of this study was to describe the temporal trend of tuberculosis cases according to sex and age group and evidence the level of disease before the Covid-19 pandemic in a TB high endemic city. METHODS: This was a time series study carried out in a city in northeast Brazil. The population was composed of cases of tuberculosis, excluding those with HIV-positive status, reported between the years 2002 and 2018. An exploratory analysis of the monthly rates of tuberculosis detection, smoothed according to sex and age group, was performed. Subsequently, the progression of the trend and prediction of the disease were also characterized according to these aspects. For the trends forecast, the seasonal autoregressive linear integrated moving average (ARIMA) model and the usual Box-Jenkins method were used to choose the most appropriate models. RESULTS: A total of 1620 cases of tuberculosis were reported, with an incidence of 49.7 cases per 100,000 inhabitants in men and 34.0 per 100,000 in women. Regarding the incidence for both sexes, there was a decreasing trend, which was similar for age. Evidence resulting from the application of the time series shows a decreasing trend in the years 2002-2018, with a trend of stability. CONCLUSIONS: The study evidenced a decreasing trend in tuberculosis, even before the Covid-19 pandemic, for both sex and age; however, in a step really slow from that recommended by the World Health Organization. According to the results, the disease would have achieved a level of stability in the city next years, however it might have been aggravated by the pandemic. These findings are relevant to evidence the serious behavior and trends of TB in a high endemic scenario considering a context prior to the Covid-19 pandemic.

INTRODUCTION: Tuberculosis remains a major health problem worldwide, killing thousand adults and children every year mainly in developing countries as Brazil. The disease is socially determined, caused mainly by inequalities as overcrowding, bad conditions of housing, unemployment, and limited access to health care. The aim of this study was to identify the social inequalities associated with the onset of tuberculosis in disease-prone territories in a city from the Northeast. METHODOLOGY: This was an ecological study, which has gathered patients diagnosed with tuberculosis through secondary data source in a city from the northeast of Brazil. The GAMLSS statistical model has been applied considering as response variable the count of Tuberculosis cases and the independent variable, the social conditions. The double Poisson distribution was considered in the analysis. The best model fitted was selected according the Akaike information criterion value. For all tests, the p value < 0.05 was considered as statistically significant. RESULTS: 460 patients with diagnosis of tuberculosis were identified, which represents an incidence of 36.3 cases/100,000 in males and 20.7 cases/100,000 in females. Regarding social inequality associated with tuberculosis, income (households with per capita income between 1/8 and 3 minimum wages), gender and age (Proportion of males under 15 years of age) were associated with the disease. CONCLUSIONS: The findings evidenced the social determinants associated with tuberculosis, with a greater occurrence of the disease in areas with mostly male children and low-income families, these issues must be managed within and beyond the health sector, which is mandatory for the Tuberculosis elimination.

OBJECTIVES: to identify risk clusters for the occurrence of tuberculosis and its treatment outcomes. METHODS: ecological study, in a city in Maranhão, using data from the Notifiable Diseases Information System. Point density analysis and isotonic scanning techniques were used to identify areas with the highest occurrence of treatment outcomes and identify risk areas for possible tuberculosis cases. RESULTS: most tuberculosis cases occurred in the male, adult, brown-skinned population. Also, most of the reported cases were classified as pulmonary and as new cases that progressed to a cure. The areas with the highest density of cure, death and abandonment are located in the central region of the city. CONCLUSIONS: the central region of the urban area of the city, with high demographic density and poor sanitary and socioeconomic conditions, presented a greater cluster of tuberculosis cases.

BACKGROUND: To evaluate the effectiveness of a rapid molecular test for the detection of tuberculosis (TB) and to predict the rates of disease in a municipality of Brazil where TB is endemic. METHODS: An ecological study was carried out in Ribeirão Preto-SP on a population of TB cases notified between 2006 and 2017. Monthly TB incidence rates and the average monthly percentage change (AMPC) were calculated. In order to identify changes in the series, the breakpoint technique was performed; the rates were modelled and predictions of the incidence of TB until 2025 were made. RESULTS: AMPC showed a fall of 0.69% per month in TB and human immunodeficiency virus (TB-HIV) co-infection, a fall of 0.01% per month in general and lung TB and a fall of 0.33% per month in extrapulmonary TB. With the breakpoint technique, general and pulmonary TB changed in structure in late 2007, and extrapulmonary TB and TB-HIV co-infection changed in structure after 2014, which is considered the cut-off point. The IMA(3) models were adjusted for general and pulmonary TB and TB-HIV co-infection, and the AR(5) models for extrapulmonary TB, and predictions were performed. CONCLUSIONS: The rapid molecular test for TB is the method currently recommended by the WHO for the diagnosis of the disease and its main advantage is to provide faster, more accurate results and to already check for drug resistance. It is necessary that professionals encourage the use of this technology in order to optimize the diagnosis so that the treatment begins as quickly as possible and in an effective way. Only by uniting professionals from all areas with health policies aimed at early case identification and rapid treatment initiation it is possible to break the chain of TB transmission so that its rates decrease and the goals proposed by the WHO are achieved.

This study aimed to analyze the discourses of patients who were diagnosed with multidrug-resistant tuberculosis, the perception of why they acquired this health condition and barriers to seeking care in a priority city in Brazil during the COVID-19 pandemic. This was an exploratory qualitative study, which used the theoretical-methodological framework of the Discourse Analysis of French matrix, guided by the Consolidated Criteria for Reporting Qualitative Research. The study was conducted in Ribeirão Preto, São Paulo, Brazil. Seven participants were interviewed who were undergoing treatment at the time of the interview. The analysis of the participants' discourses allowed the emergence of four discursive blocks: (1) impact of the social determinants in the development of multidrug-resistant tuberculosis, (2) barriers to seeking care and difficulties accessing health services, (3) perceptions of the side effects and their impact on multidrug-resistant tuberculosis treatment, and (4) tuberculosis and COVID-19: a necessary dialogue. Through discursive formations, these revealed the determinants of multidrug-resistant tuberculosis. Considering the complexity involved in the dynamics of multidrug-resistant tuberculosis, advancing in terms of equity in health, that is, in reducing unjust differences, is a challenge for public policies, especially at the current moment in Brazil, which is of accentuated economic, political and social crisis. The importance of psychosocial stressors and the lack of social support should also be highlighted as intermediary determinants of health. The study has also shown the situation of COVID-19, which consists of an important barrier for patients seeking care. Many patients reported fear, insecurity and worry with regard to returning to medical appointments, which might contribute to the worsening of tuberculosis in the scenario under study.

BACKGROUND: A diagnosis of tuberculosis (TB) does not mean that the disease will be treated successfully, since death may occur even among those who are known to the health services. Here, we aimed to analyze patient survival time from the diagnosis of TB to death, precocious deaths, and associated factors in southern Brazil. METHODS: We conducted a longitudinal study with patients who were diagnosed with TB and who died due to the disease between 2008 and 2015 in southern Brazil. The starting point for measuring survival time was the patient's diagnosis date. Techniques for survival analysis were employed, including the Kaplan-Meier test and Cox's regression. A mixed-effect model was applied for identifying the associated factors to precocious deaths. Hazard ratio (HR) and odds ratio (OR) with 95% confidence intervals (95% CI) were estimated. We defined p value <0.05 as statistically significant for all statistics applied. RESULTS: One hundred forty-six patients were included in the survival analysis, observing a median survival time of 23.5 days. We observed that alcoholism (HR=1.55, 95% CI=1.04-2.30) and being male (HR=6.49, 95% CI=1.03-2.68) were associated with death. The chance of precocious death within 60 days was 10.48 times greater than the chance of early death within 30 days. CONCLUSION: Most of the deaths occurred within 2 months after the diagnosis, during the intensive phase of the treatment. The use of alcohol and gender were associated with death, revealing inequality between men and women. This study advanced knowledge regarding the vulnerability associated with mortality. These findings must be addressed to fill a gap in the care cascades for active TB and ensure equity in health.

Utilizó técnica de análisis de densidad de puntos y de barredura isotónica, para identificación de áreas de mayor ocurrencia de los desfechos en el tratamiento e identificación de áreas de riesgo para los casos de tuberculosis. Resultados: mayoría de los casos de tuberculosis ocurrió en personas del sexo masculino, adultos, de la raza parda. También, la mayor parte de los casos notificados se clasificó como pulmonar y se refirió a casos nuevos, progresando hacia la cura. Áreas con mayor densidad de cura, óbito y abandono están ubicadas en la región central del municipio. Conclusiones: la región central del área urbana de la ciudad, con alta densidad demográfica y precarias condiciones sanitarias y socioeconómicas, presentó mayor aglomerado de casos de tuberculosis.

Utilizou-se técnica de análise de densidade de pontos e de varredura isotônica, para identificação das áreas de maior ocorrência dos desfechos no tratamento e identificação das áreas de risco para os casos de tuberculose. Resultados: a maioria dos casos de tuberculose ocorreu em pessoas do sexo masculino, adultos, da raça parda. Também, a maior parte dos casos notificados classificou-se como pulmonar e se referiu a casos novos, progredindo para a cura. As áreas com maior densidade de cura, óbito e abandono estão localizadas na região central do município. Conclusões: a região central da área urbana da cidade, com alta densidade demográfica e precárias condições sanitárias e socioeconômicas, apresentou maior aglomerado de casos de tuberculose.

As doenças crônicas não transmissíveis constituem a maior carga de morbimortalidade no mundo sendo responsáveis por 63% das mortes globais. No Brasil, ainda representam a maior mortalidade do país. Com isso, o objetivo do estudo foi identificar áreas de risco e proteção para a mortalidade prematura por doenças crônicas não transmissíveis segundo a vulnerabilidade social no município de Ribeirão Preto de 2010 a 2014. Tratou-se de estudo ecológico, quantitativo que por meio do método de varredura espacial calculou-se o risco relativo e intervalo de confiança de 95% segundo dados de vulnerabilidade social. Identificaram cinco áreas com significância estatística, sendo duas áreas de alto risco com predomínio de setores censitários classificados como vulnerabilidade baixa, vulnerabilidade média e vulnerabilidade alta em ambientes urbanos. E três áreas de proteção com predominância de setores censitários classificados como baixíssima vulnerabilidade. Foi possível identificar as áreas de risco ou proteção para a mortalidade prematura por DCNT pode contribuir para a elaboração de estratégias inovadoras efetivas na redução da carga destas doenças para o SUS.


Chronic non-communicable diseases constitute the highest burden of morbidity and mortality in the world and is responsible for 63% of deaths worldwide. In Brazil, they still represent the highest mortality in the country. Thus, the objective of the study was to identify areas of risk and protection for premature mortality from chronic non-communicable diseases according to social vulnerability in the city of Ribeirão Preto from 2010 to 2014. This was an ecological, quantitative study in which, through the method of spatial scanning, the relative risk and 95% confidence interval were calculated according to social vulnerability data. Five areas of statistical significance were identified, with two high-risk areas being predominantly of census sectors classified as low vulnerability, medium vulnerability, and high vulnerability in urban environments, and three protection areas with being predominantly of census sectors classified as extremely low vulnerability. It was possible to identify areas of risk or protection for premature mortality due to CNCDs, which can contribute to the development of effective innovative strategies to reduce the burden of these diseases for SUS.
